Item No.: 08
Site: Potable Water-Potable Water - Backflow Prevention
Violation: A non-continuous pressure-type backflow prevention device (atmospheric vacuum breaker) was installed on the potable water line supplying the officer laundrette on deck 8.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Site: Potable Water-Potable Water - Backflow Prevention
Violation: A non-continuous pressure-type backflow prevention device (atmospheric vacuum breaker) was installed on the potable water line supplying the officer laundrette on deck 8.
Recommendation: Ensure that an atmospheric vacuum breaker is installed only in the supply line on the discharge side of the last control valve.

Site: Potable Water-Potable Water
Violation: Non-potable water lines were painted blue. This was noted in the whirlpool pump room and the incinerator room.
Recommendation: Ensure that potable water piping and fittings are painted or stripped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Site: Potable Water-Potable Water - Backflow Prevention
Violation: The air gaps for the mineralizer backwash discharge lines were not twice the diameter of the lines.
Recommendation: Ensure that air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 3 cm (1 inch).

Site: Potable Water-Potable Water - Cross-connection Control
Violation: Not all of the connections to the potable water system were listed in the comprehensive cross-connection control program. The connections in the deck 7officer laudrette and the steam generators were not listed.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Violation: The follow-up of close contacts of gastrointestinal illness cases were not documented by medical staff.
Recommendation: Document the follow-up of asymptomatic close contacts of gastrointestinal illness cases.

Violation: The large ice machine was posted out of order "for a long time."
Recommendation: Repair the large ice machine.

Violation: Excess sealant was noted on the interior of the ice cuber section, making cleaning difficult.
Recommendation: Remove excess sealant from the ice machine.

Violation: The paint was peeling on the metal shelving units, making cleaning difficult.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: The dataplate on the mechanical potwash was deteriorated, making cleaning difficult.

Violation: Fiberboard was used beneath the stacked bottles of water. This board was porous, torn and otherwise not easily cleanable.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: The mounted final rinse pressure gauge on the in-use conveyor dishwasher recorded 32 psi.
Recommendation: Repair the final rinse pressure gauge on the conveyor dishwasher.

Violation: The disposable paper towel dispenser for the handwash station was posted over the food preparation counter.

Violation: There was a gap under the stainless steel profile strip at the end of the bar. There was a gap at the threshold of the bar.

Violation: The deck of the technical compartment beneath the toaster section was soiled.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Violation: The deck-cabinet junctions of the waiter stations were not coved. This is a repeat from the last inspection.
Recommendation: Ensure that bulkhead and deck junctures are coved (including galleys, pantries, deck/counter junctures at buffets, bars, waiter stations, and dining room work counters).

Violation: A leak was noted at the base of the faucet of the single compartment sink. This item was corrected during the inspection.
Recommendation: Ensure that a plumbing system in a food area is maintained in good repair.

Violation: The two fluorescent lights on the deckhead were uncovered.
Recommendation: Ensure that light bulbs are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles.

Site: Housekeeping-Housekeeping - Infection Control
Violation: The Deck 7 women's toilet room was not equipped to allow for exit without the use of bare hands. This was corrected.
Recommendation: Equip public toilet facilities so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.

Site: Housekeeping-Outbreak Prevention and Response Plan
Violation: In the outbreak plan the areas under concessions (spa, gym, casino and others) were to be sanitized with a chlorinated cloth at the end of the day during an outbreak and not continuously as is required. It was not always clear in the plan that public areas were to be disinfected continuously throughout the day when 2% or greater illness rates were reached in passengers or crew members.
Recommendation: Ensure that when the cumulative proportion of cases of gastrointestinal illness among passengers or crew members is 2%, the infection control response includes cleaning and disinfecting all public areas, including handrails and restrooms, on a continuous basis until the proportion decreases to < 2%.
