Incidence of a TTP-HUS-like syndrome in patients hospitalized with AIDS.

A TTP-HUS-like syndrome has been seen in some clinical trials in AIDS patients. We wished to determine the prevalence of and factors associated with this syndrome in HIV-infected patients hospitalized at the Johns Hopkins Hospital. 350 consecutive patients admitted from May 1996 through Feb.1997 were assessed for evidence of clinical features of TTP-HUS. Patient characteristics were: men 69%, African-American 82%, injecting drug users 59%, median CD4=36, median HIV-RNA= 94,000, CDC-stage C disease 72%. The prevalence of TTP-HUS features was: moderate (fragmented RBCs on smear seen in most high-power fields) schistocytosis 24%, anemia 56%, thrombocytopenia 29%, fever 5%, neurologic dysfunction 66%, renal insufficiency (creatinine greater than l.5 mg/dl) 68%. Schistocytosis occurred with anemia and thrombocytopenia in 13% and with neurologic dysfunction, fever, or renal insufficiency in 7% of patients. Factors associated with TTP-HUS-like features were CD4 less than 50 cells/mm(3) and Stage C disease. No specific drug or diagnosis was associated with schistocytosis or any combination of features. A TTP-HUS-like syndrome is common in hospitalized AIDS patients unrelated to pharmaceutical therapy. This relatively high prevalence is important to be aware of when assessing the potential adverse effects of new drug therapies being assessed in clinical trials of AIDS patients.
