Self-Selection Process Benefits

Displaced Worker Medical Benefit Program Operations Manual (pdf)

If you departed the Lab under conditions of the Self-Selection Program on Jan 10, 2008 you were eligible for continuation of Medical, Dental and Vision coverage under the Displaced Worker Medical Benefit Program (DWMBP). If you were not eligible for other coverage, you could choose to continue any or all of the above mentioned benefits for up to two years before COBRA continuation commenced. Pricing for each year is based on a different formula.

  • Year 1: the continuant is responsible for the same premium as an active employee
  • Year 2: the continuant is responsible for the same premium as an active employee, plus 50% of the employer premium
  • Year 3: the continuant may continue coverage under COBRA, and is responsible for 102% of the premium

Continuation of the Medical, Dental and Vision coverage is administered by United Healthcare Direct Bill. Please contact UHC Direct Bill for any billing questions or to report a qualified change in status while participating in the DWMBP (for example: marriage, birth, divorce). If you become eligible for Medical, Dental or Vision coverage through any other source you must immediately notify UHC Direct Bill and cancel the equivalent DWMBP coverage (for example: if you are eligible for Medical and Dental through a new employer you must cancel your DWMBP coverage in Medical and Dental, but you may continue Vision).

If you are eligible for Retiree Medical and Dental (TCP1 or TCP2)

Then you are not eligible for DWMBP Medical and Dental. You will need to sign up for you Retiree Medical and Dental benefits through Hewitt Associates, Your Benefit Resources. You may continue Vision under DWMBP until the eligible time-frame is exhausted or you become eligible for Vision coverage through any other source.
