Agreements on and execution of AIDS home care in Groningen, The Netherlands.

In 1986 a tutorial has been established by managers of the home health care agency (the Green Cross) and the University Hospital of Groningen (A.Z.G.). The special task was, to create and implement AIDS home care and to prevent unnecessary hospitalization. OBJECTIVE. The aim for both organizations is: to stimulate out-patient treatment and home care. AGREEMENTS. Sharing responsibility: follow up, 24 hours emergency care, an in-patient multidisciplinary intake, to remove and replace contaminated trash, update AIDS education, coordination by Clinical Nurse Specialist (C.N.S.). EXECUTION. 5 Years population 140 HIV positive patients. 30 (21%) needed home care, 4 (2.9%) needed a nursing home, 2 (1.4%) a psychiatric hospital. RESULTS. Failures: home care impossibility (medical impossibility) 20%, satisfactory 80%. Complete execution by the C.N.S.: 27 (90%). Incomplete execution not by C.N.S.: 3 (10%). CONCLUSION. Agreements on AIDS home care should be made on top management level by the involved organizations. The C.N.S. coordinates and evaluates the home care process. The multidisciplinary team should meet on a regular basis to discuss and up date the individual plan of care.
