OBJECTIVE: To survey stable couples with discordant HIV serostatus. METHODS: In heterosexual couples living together and having had sexual intercourse for more than twelve months, the seronegative partner was tested regularly. We compared couples in whom the negative partner seroconverted or remained seronegative. RESULTS: Seventy-four couples have been followed since 1985. Sex of initial seropositive patient: 33 females and 41 males, sex ratio: 1.2. Intercurrent STD: 5 (7%); intercurrent pregnancies: 26 out of 23 couples (31%) induced abortions: 4 (7.8%), delivery: 22 (32.3%). Seroconversions: 15/74 (20%); 3 males/33 (9%), 12 females/41 (29.3%). Average length of seroconversion: 23 months(min:6, max 60). Clinical and biological characteristics of propositus at time of partner's seroconversion: 13 G2/G3 p24 antigenemia negative (93%), 1 G4C1 p24 ag positive. Only 28 couples (38%) used condoms. Seroconversion among couples non using condoms after an average 35 months of regular sexual intercourse: 15/46 (33%). CONCLUSION: In Martinique, in spite of a satisfactory medical follow-up, the high rate of seroconversion (20%) in heterosexual couples with discordant HIV serostatus, underlines the failure of our safe-sex counselling policy (condom use).
