Background: Defects of mucosal, cell-mediated and humoral immunity predispose HIV-infected patients to acquire bacterial infections. Nonetheless, bacterial infection of the parotid gland remains a rare entity in this population. Case: A 24 year-old man with a 5-days history of a pustule lesion (folliculitis) on the front-head, presented with high fever and a progressive swelling of the front head area, nose, periorbital region and the parotid glands. He was HIV-positive without prior history of an AIDS-defining illness, on treatment with 3TC, d4T, and nevirapine, and a recent CD4 cells count of 361 cells/mm3 and an HIV-1 viral load of 8,028 copies/ml. An ultrasound of the parotid gland disclosed increase size of both glands with hypoecogenic lesions inside; a CT showed the same lesions with contrast enhancement, highly compatible with abscess formation. The patient underwent repeated surgical drainage and received intravenous clindamycin and amikacin for three weeks, with complete resolution of the clinical picture. Discussion: Acute bacterial parotitis typically involves organisms from the mouth that reach the gland ascending through the Stenon conduct, and is associated with decreased outflow of saliva. This case describes an unusual presentation of bilateral suppurative parotitis that resulted from lymphatic or blood spread of organisms from another site, such as folliculitis on the front head.
