NCI Multicenter Study of AIDS in Hemophiliacs, Bethesda, MD USA

OBJECTIVE: To study the sexual transmission of HCV by multitransfused hemophiliacs (H) coinfected with HIV. METHODS: Sera stored frozen from 239 female sexual partners (FSP) of H from 8 U.S. centers located in PA, NY, OH and LA were tested for antibodies for HCV and HIV by EIA. 48 FSP of 46 H were tested at one center where HCV tests were available on 37 H. RESULTS: Ten (4%) of 239 FSP were anti-HCV+, 2 (1%) were indeterminate, and 227 (94%) were anti-HCV-. Anti-HIV prevalence among the FSP was 25/239 overall and 25 (12%) of 206 FSP of anti-HIV+ H. 4 FSP were coinfected with HCV and HIV, substantially more than would be expected by change (p = .01 Fisher's exact test). The mean CD4 count in their 4 H partners was 125 cells/mul. TABULAR DATA, SEE ABSTRACT VOLUME. At one center, 34 (92%) of H with FSP were anti-HCV+. CONCLUSIONS: (1) The prevalence of HCV infection in female sexual partners of multitransfused hemophiliacs is at least 4%. (2) HIV is transmitted more frequently than HCV by men infected with both HIV and HCV (12% vs. 4%). (3) HCV transmission is 6 times higher (4/25 = 16% vs. 6/214 = 2.8%, p=.01) in immune deficient men who cotransmit HIV, suggesting that HIV-related immune deficiency may allow active replication and transmission of HCV.
