16. Description of Occurrence:


On March 30, 1998, the first line supervisor of the Cryogenic
Refrigerator complex discovered oil stained soil and spillage
at the oil mist separator. The last time the area was
inspected was on Friday, March 27, 1998, at approximately
1600.

The spill of a maximum of 20 gallons was due to failure of a
sight glass on the oil mist recovery system.

The spill was immediately reported to the Laboratory emergency
telephone number (x2222). The supervisor placed absorbent
pads on the areas of oil contamination and a cleanup crew was
contacted to remove the oil stained soil.

Failure of an O-Ring seal on the site glass

The piping system will be redesigned and all site glasses will
be removed.
