Reporting Materials

Adverse transfusion reactions suspected to be due to bacterial contamination of blood or blood products should be reported through standard operating procedures and FDA regulations; although the BaCon Study has been completed, study materials and report forms will continue to be available through the links below to assist with routine reporting.
