Meta-analysis of diagnostic accuracy of ultrasonographic screening in prenatal detection of Down syndrome.

OBJECTIVE: To analyze the diagnostic accuracy of ultrasound markers, single or in combination, for the prenatal detection of Down Syndrom (DS). METHOD: Meta-analysis of diagnostic tests using the random and fixed effects model. Data collection has been performed by two independent researchers following a specific protocol. RESULTS: Overall, 48 studies were identified according to predetermined selection criteria. The global values for the diagnostic accuracy, in terms of overall sensititivity and specificity, of the ultrasound markers were the following: Random Effects Model (REM) (Sensitivity (CI 95%), Specificity (CI 95%)), Fixed Effects Model (FEM) (Sensitivity (CI 95%), Specificity (CI 95%)): NT1 (65% (50-78%), 98% (94-99%))(66% (59-72%), 91% (90-91%)); NT2 (42% (33-51%), 99% (98-100%))(42% (36-48%), 97% (96-98%)); FL (32% (24-40%), 92% (89-94%))(32% (28-37%), 88% (87-89%)); NT&FL (61% (48-73%), 91% (89-94%))(61% (51-70%), 91% (90-93%)). CI: Confidence Interval; NT1: nuchal thickness in the first trimester; NT2: nuchal thickness in the second trimester; FL: femur length; NT&FL: combination of both markers in the second trimester of pregnancy. CONCLUSIONS: Given the lack of a prenatal screening test for the DS in the first trimester pregnancies, ultrasound screening could be adopted, by means of the measure of the nuchal thickness, once the ethical and economic consequences of the procedure were assessed. Further research should consider issues such as quality of study design and combinations with biochemical markers and maternal age.
