Infective Endocarditis (IE) Due to HACEK: Report from the International Collaboration on Endocarditis (ICE).

BACKGROUND: Haemophilus, Actinobacillus, Cardiobacterium, Eikenella, and Kingella (HACEK) cause a small percentage of cases of native valve and late-onset prosthetic valve IE. This study was designed to investigate the clinical features of IE due to HACEK species in a multinational cohort. METHODS: The ICE group has been formed to study IE. 7 of 25 ICE centers have existing IE databases representing > 3000 total cases. Individual databases were combined into a single large database using a core set of variables with standard definitions. Cases of IE due to HACEK were identified. RESULTS: Of 2268 definite cases of IE from 7 sites in 5 countries, 40 (1.8%) were due to HACEK. The median age of patients was 47.5 years. Of 33 cases in which it was recorded, 9 (27.3%) involved prosthetic valves. Haemophilus species were the cause in 21 cases (52.5%) of IE due to HACEK. 8 cases (20%) were caused by Actinobacillus, 6 (15%) by Cardiobacterium, and 1 by Eikenella. In 4 cases the HACEK species was not specified. Valvular site of involvement was specified in 25 cases: 64% were mitral, 36% were aortic. Heart failure occurred in 6 cases, intracardiac abscesses in 4, and stroke in 2. Recorded rates of surgery during the episode of IE were 60% (15 of 25), and recorded overall mortality was 11.1% (4 of 36). CONCLUSIONS: In this large multinational cohort, HACEK species were responsible for a relatively small number of cases of IE. Cases were characterized by high rates of surgery, but relatively low overall mortality.
