Itraconazole for the treatment of HIV-associated eosinophilic folliculitis.

OBJECTIVE: To investigate the efficacy of itraconazole for the treatment of HIV-associated eosinophilic folliculitis (EF). METHODS: 30 HIV-positive patients were entered into the study. The mean age was 39.1. The mean CD4 cell count was 67.6. At Center 1, patients whose histories and physical finding were compatible with the diagnosis of EF had skin biopsies performed and were begun on itraconazole therapy in a dose of 200 mg twice a day. Regardless of the dermatohistopathology result, all patients clinically diagnosed with EF were treated with itraconazole. At center 2 all patients presented with an urticarial, papular eruption of the face and/or trunk. A skin biopsy was performed. After histologically confirming the clinical diagnosis of EF, treatment with itraconazole was begun in doses varying from 100 mg to 300 mg daily. Therapy was discontinued at both centers at the determination of treatment success, treatment failure, or an adverse event. RESULTS: At doses varying from 100 mg to 400 mg of itraconazole daily, a complete response was obtained in 18/30 (58%), a partial response in 3/30 (10%), and failures in 4/30 (13%). Five of 30 patients (17%) had an adverse reaction, cutaneous eruptions in 4 and elevated liver enzymes in one. Eight week follow-up in 10 patients showed that 2 remained clear off treatment, 6 remained clear on treatment, and 2 were partially clear on treatment. CONCLUSION: Itraconazole has shown impressive benefit in the therapy of EF in patients with HIV disease.
