1. Does the seat have a label showing it was manufactured after January 1, 1981?
__Yes Continue      
__ No Throw the seat away! It is no longer safe and should not be used.      
2. Does the seat have a label showing that it meets all Federal Motor Vehicle Safety Standards? __Yes Continue      
__ No Throw the seat away! It is no longer safe and should not be used.      
3. Has the seat been recalled for safety defects? (To find out, call the DOT Auto Safety Hotline* and give them the model number and date of manufacture of the seat.) __ No Continue      
__Yes

Have the defects been corrected?

__ Yes Continue
__ N Throw the seat away! It is no longer safe and should not be used.

4. Has the seat been involved in a vehicle crash? __ Yes Throw the seat away! It is no longer safe and should not be used.
__ No Continue

     
5. Are there any cracks, bends, or breaks in the plastic shell or metal frame? __ Yes Throw the seat away! It is no longer safe and should not be used.      
__ No Continue

     
6. Do you have a copy of the manufacturerís instructions? __ Yes Continue      
__ No Call the Auto Safety Hotline* for telephone number of manufacturer to request instructions.

     
7. Does the seat have all its parts: harness straps, retainer clips, padding, shield, tether, straps, and bolts? __ Yes Continue      
__ No Throw away - or obtain replacements from the manufacturer.

     
8. Are the harness straps worn or frayed? __ Yes Throw away - or obtain replacements from the manufacturer.      
__ No Continue

     
9. Does the buckle area show signs of rust? __ Yes Throw away - or obtain replacements from the manufacturer.      
__ No Continue

     
10. Does the buckle latch mechanism fasten securely, remain fastened, and unfasten smoothly? __ Yes Use the seat!      
__ No Throw away - or obtain replacements from the manufacturer.
