Steps to repeat the problem:
1. Click "Username" input field and press 'a' on the keyboard to enter some value.
2. Use the "Folder" select to change the value from "Bar" to "Foo"
3. See the background Console view there is an error (Error: getUserNameAndPasswordFields: input element required) that appears as soon as you change the value of the Folder select.
